Uncertified Roof Anchors

Any anchorage a worker ties off to must hold 5,000 pounds per attached person (§1910.27(b)), or be engineered with a 2:1 safety factor under a qualified person. Older buildings often have anchors that were never load-tested, were added informally, or have corroded — and a window-washing or maintenance contractor won't work until they're certified.
